Ensuring all clinical groups and directorates develop, implement and maintain robust local risk management practices, ensuring compliance with the requirements of the Trust’s Risk Management Policy.
Promoting and developing a fair and open approach where staff report incidents in a timely manner.
Managing the Incident Reporting Policy and associated procedures throughout the Trust, as the basis for reporting and learning to mitigate incidents.
Providing support and advice to clinical groups and directorates in carrying out investigations of incidents and near misses including the identification and implementation of action plans and control measures to prevent incident recurrence.
Ensuring proper investigation and follow-up of serious incidents, providing briefings to the appropriate committees, Executive Directors, Trust Board, etc.
Providing expert advice and leadership on matters relating to risk assessment, management and the promotion of assurance within the Trust.
Leading on further developing and maintaining the Trust-wide Risk Register, ensuring that processes are in place for all entries and ratings to be kept up to date, actions to manage risks are undertaken within deadlines set and an audit trail is kept.
